The district nurses
work together with the GPs, patients
and their carers in the
community where they holistically
assess healthcare needs and develop appropriate packages of care for
housebound patients.
The health visitors also work with
the GPs, midwives and other agencies
to offer support to families and
promote their emotional and physical
well-being through individual and
group contacts.
